Care Consultation Services
Our dedicated, experienced social workers are available to work and talk with seniors, their caregivers and family members one-on-one. We can meet virtually, in your home or in our office. These consultations can be a great neutral space to address topics like:

Individual needs of an aging senior and facilitate proactive planning

How to help older adults remain living at home as long as possible and connected to the community

Assess levels of caregiver stress and provide access to local, supportive services

Provide tools and educational opportunities for caregivers and concerned family members

Support informed, thoughtful decision making when it comes to addressing the full continuum of needs, such as housing, home care assistance, transportation and housekeeping

Help develop a collective action plan that shares responsibilities within a family dynamic
Care consultation services are offered on a sliding fee scale basis. Our goal is to keep costs low to sustain access and availability to all who seek our services. Generous donors enable The Center to provide these services at reduced rates.
No one is denied service because of inability to pay.
